Interaction

Before starting Topamax, be sure to give your physician a list of all the drugs you are using, including prescription, over-the-counter (OTC), vitamins, herbal supplements, and recreational drugs.Using carbamazepine (Tegretol) or phenytoin (Dilantin) may increase the elimination of Topamax from your body, making it less effective.The following drugs might increase your risk of kidney stones and metabolic acidosis:

  • Acetazolamide (Diamox)
  • Dorzolamide (Trusopt)
  • Methazolamide (Neptazane)
  • Dichlorphenamide (Daranide)

If you combine Topamax with valproic acid (Depakote, Depakote ER, Depakene, Depacon, Stavzor) this could raise your blood ammonia levels, causing hypothermia.

Uses

  • Take this medicine only as directed by your doctor. Do not take more of it, do not take it more often, and do not take it for a longer time than your doctor ordered. If too much is taken, it may become habit-forming (causing mental or physical dependence).
  • This medicine comes with a Medication Guide. Read and follow the instructions carefully. Ask your doctor if you have any questions.
  • Take this medicine once a day in the morning, with or without food. Avoid taking this medicine in the evening to prevent trouble sleeping.
  • Swallow the extended-release capsule whole. Do not crush, break, or chew it.
  • Carefully follow your doctor's instructions for a reduced-calorie diet plan and regular exercise. Talk with your doctor before starting any exercise program.

Dosges

In the case of adults and children 10 years or older, the dose may be increased by 100 mg in week five and six if Topamax is used by itself. The final recommended adult dose is 400 mg, taken in two divided doses daily.Children should start with up to 25 mg taken each night for the first week, with the dose being increased at 1 or 2 week intervals. The maximum maintenance dose is based on weight.

Warning

Topamax can lead to serious eye problems that, if left untreated, could cause permanent vision loss. These include a sudden decrease in vision that may be accompanied by eye pain and redness, as well as a blockage of fluid in the eye resulting in secondary angle closure glaucoma.Topamax might decrease sweating and increase your body temperature resulting in a fever. Some people may need to be hospitalized if this occurs.
