How Does Alprostadil Treat Erectile Dysfunction?

Alprostadil  is a medicine that men take for ED (erectile dysfunction). It’s only available by prescription.

This drug makes the blood vessels expand. That boosts blood flow throughout the body, including the penis, so it helps men with ED have an erection.

You can get alprostadil injected directly into the penis. Or you can use it as suppository, placing it into the opening at the tip of the penis.

Injectable alprostadil is sold under the brand names Caverject, Edex, and Prostin VR. The suppository form is called “MUSE” (which stands for Medicated Urethral System for Erection).

What types of injections are used to treat ED?

Oral medications don't work for everyone. Many men may be prescribed medicines that go directly, via injection, into the penis. Injection therapy may use a single drug or a combination of drugs. Single drug injection therapy uses alprostadil, a type of prostaglandinE1 (PGE1), and is called either Caverject® or Edex®. Combination therapies, called "bi-mix" (for 2 drugs mixtures) or "tri-mix" (for 3 drugs mixtures), are a mixture of either two, or all three, of the following most commonly used drugs: papaverine, phentolamine and/or alprostadil. Not all bi-mix or tri-mix medications are identical - the amount of the individual drugs may vary from pharmacy to pharmacy.

Alprostadil is a vasoactive agent that is a synthetic version of the chemical prostaglandin E. This chemical helps relax the smooth muscle tissue in the penis to enhance blood flow needed for an erection.

Specially designed vacuum devices to produce erections have been used successfully for many years. Vacuum devices are safe, relatively inexpensive, and reliable. Vacuum devices do not require surgery. Vacuum devices are available over the counter or by prescription. It is important to make sure that the vacuum device have a mechanism to prevent too high of a vacuum (negative pressure).

The typical vacuum device consists of a plastic cylinder that is placed over the penis, tension rings of various sizes, and a small hand or battery operated pump. Air is pumped out of the cylinder, causing a partial vacuum (negative pressure), which increases blood flow into the penis and creates the erection.
