If you are allergic to any of the ingredients of Rivamer 3Mg Capsule or to carbamate derivatives like meprobamate, do not use it. It is also not to be used if you use another form of it, or are taking acebutolol, atenolol, betaxolol, bisoprolol. If you have dementia that is not caused by either Parkinson’s or Alzheimer’s, or if you have a scheduled surgery coming up, let your doctor know.

Rivamer 3Mg Capsule is taken by mouth with the morning and evening meals. It is important that you take it regularly so as to reap most benefit out of it. The dosage usually starts out low and is then increased after about two to four weeks.

Side effects of Rivamer 3Mg Capsule include weight loss, vomiting, weakness, dizziness, diarrhea, and increased sweating. If you encounter mood changes, bloody or black stools, fainting, decreased coordination, find a doctor immediately.

4. How To Take Rivamer 3mg Capsule?

You should take your Rivamer capsules 3mg swallowed whole with a glass of water, twice daily, with food, preferably at morning and evening meal times.  Your doctor may increase your dose depending on your response and your condition.  You should continue to take your Rivamer capsules 3mg for as long as recommended by your doctor as they help to slow the progression of Alzheimer's disease but they do not cure it.

5. Mechanism Action Of Rivamer 3mg?

Rivamer capsules 3mg contain rivastigmine, a cholinesterase inhibitor that is used to treat Alzheimer's Disease. 

The loss of cognitive function (attention, thinking, reasoning, learning and memory) that is associated with Alzheimer's Disease is caused by degeneration of brain cells, particularly those that produce the neurotransmitter acetylcholine (a neurotransmitter is a chemical that allows nerve cells to communicate).  Acetylcholine is continually released by brain cells in specific areas of the brain that control cognitive function and it is then degraded by the enzyme cholinesterase to prevent its build up in the synapses (gaps between neurones). 

A feature of the brain associated with the development of Alzheimer's disease is the appearance of two abnormal structures.  Plaques are deposits of protein fragments called beta-amyloids that build up between neurons (nerve cells) and are generated by the destruction of amyloidogenic beta-amyloid-precursor protein (APP), the fragments of which form the amyloid plaques. 

The other structure consists of tangles of twisted fibres of another protein called tau that build up inside dying nerve cells.  Plaques and tangles are thought to be responsible for damaging and killing nerve cells; followed by loss of connections between neurones in the brain. 

The tangles and bundles start in the inner temporal lobes and the cerebral cortex and then the spread to other regions of the brain. 

Rivastigmine in Rivamer capsules 3mg increases the level of acetylcholine in the brain by inhibiting the action of cholinesterase, which slows down the degradation of acetylcholine released by acetylcholine-producing neurones that are still functional and this increases levels of acetylcholine in the damaged areas of the brain.  It is also thought that inhibition of cholinesterase may slow the formation APP fragments and thereby of the amyloid plaques. 

Rivamer capsules 3mg help restore cognitive processes of thinking, learning and memory, and improve daily functioning in people living with Alzheimer's disease, by slowing down the progression of the disease; but they cannot prevent its progression or cure the disease.

8. Warning of Rivamer 3mg Capsule

Rivamer 3mg Capsule is probably safe to use during pregnancy.

Animal studies have shown low or no adverse effect on the foetus, however, there are limited human studies. Please consult your doctor.

Rivamer 3mg Capsule is probably unsafe to use during lactation. Limited human data suggest that the drug could represent a significant risk to the baby.

Rivamer 3mg Capsule may make you feel dizzy, sleepy, tired, or decrease alertness. If this happens, do not drive.

Rivamer 3mg Capsule is safe to use in patients with kidney disease. No dose adjustment of Rivamer 3mg Capsule is recommended.

However, inform your doctor if you have an underlying kidney disease as close monitoring is advised while you are taking this medicine.

9. Precaution of Rivamer 3mg Capsule

Apply one patch per day by pressing it firmly in place for at least 30 seconds to only one of the following locations: left upper arm or right upper arm, left upper chest or right upper chest (avoid breast), left upper back or right upper back, left lower back or right lower back.

Do not apply a new patch to that same location of skin twice within 14 days.  

Before you apply a patch, make sure that your skin is clean, dry and hairless, free of any powder, oil, moisturizer, or lotion that could keep the patch from sticking to your skin properly, free of cuts, rashes and/or irritations. Do not cut the patch into pieces.

Do not expose the patch to any external heat sources (e.g. excessive sunlight, saunas, solarium) for long periods of time. Make sure the patch does not loosen during any physical activities such as bathing, swimming or showering.

Replace the patch with a new one only after 24 hours. If you have not applied a patch for several days, do not apply the next one before you have talked to your doctor.

Take precautions if you have or had any of the following medical conditions: an irregular heartbeat, an active stomach ulcer, inflammation of the pancreas, difficulties in passing urine, seizures, asthma or a severe respiratory disease, trembling, low body weight, gastrointestinal reactions such as nausea, being vomiting and diarrhea, impaired liver function, planning to undergo surgery, dementia or other decreased mental ability that is not caused by Alzheimer or Parkinson disease.

Do not drive or operate machinery as rivastigmine may cause fainting or severe confusion. 

Tell your doctor if you are or planning to become pregnant.
