Hepcvir (Sofosbuvir) is most commonly prescribed for the treatment of Hepatitis C in adults. It is used with other medications in combination. These other medications are usually ribavirin and pegylated interferon. Sufosbuvir is able to treat genotypes 1, 2, 3, and 4 of Hepatitis C.

3. How To Take Hepcvir 400mg Tablet?

Take this medicine in the dose and duration as advised by your doctor. Swallow it as a whole. Do not chew, crush or break it. Hepcvir 400mg Tablet is to be taken with food.

4. Mechanism Action Of Hepcvir 400mg?

Hepcvir tablets 400mg contain sofosbuvir, a direct-acting antiviral agent that is used in combination with other antiviral medications to treat chronic HCV infection. 

Sofosbuvir in Hepcvir tablets 400mg is a direct-acting nucleotide analogue inhibitor that blocks the action of the HCV NS5B polymerase, an enzyme that is essential to viral replication.  It is a nucleotide prodrug that is converted to its active form, the uridine analog triphosphate (GS-461203), which is incorporated into HCV RNA by the action of NS5B polymerase. 

This action of sofosbuvir in Hepcvir L tablets causes chain termination of viral RNA synthesis thereby inhibiting viral replication.  Sofosbuvir in Hepcvir tablets is an effective treatment for HCV infection only when used in combination with at least one other antiviral medication

These include interferon, which is a chemical messenger produced by host cells in response to infection by various pathogens, including viruses that stimulates the body’s own immune response, as well as directly inhibiting viral replication in infected cells. 

Ribavirin is a synthetic ribonucleic analogue and is incorporated into viral RNA blocking RNA synthesis; however, it does not have any antiviral activity when used alone. 

The specific combination therapy regimen for HCV infection depends on the HCV genotype and the degree of liver damage due to the infection. 

The combination of Hepcvir tablets 400 mg and ribavirin or ribavirin with interferon lowers the amount of hepatitis C virus in the blood and may also lead to a cure of HCV infection over a number of weeks.

6. Warning of Hepcvir 400mg tablet

Hepcvir (Sofosbuvir) should not be taken alone. It should be taken as part of an overall treatment plan usually also containing ribavirin and pegylated interferon. Your doctor will advise you further before beginning treatment with Sofosbuvir.

Do not take Sofosbuvir if you are pregnant or trying to become pregnant. Sofosbuvir can result in birth defects, damage to the fetus, or early termination of pregnancy. It is recommended to use mulitple forms of contraception to prevent pregnancy while taking this medication and for 6 months after treatment ends, as it is vital you do not become pregnant while you or your partner are taking sufosbuvir.

Sofosbuvir can cause sever diarrhea in some cases. In you experience this, be sure to maintain your fluid intake to sufficient levels to prevent dehydration.

7. Precaution of Hepcvir 400mg tablet

Hepcvir (Sofosbuvir) should be taken for a period of 12 weeks unless otherwise instructed by your doctor. The other medications being taken along with sufosbuvir should be taken for the same length of time. It is better to take the medicines at the same time, and to do so at the same time each day. You can take Hepcvir in the morning (to reduce insomnia) and to take it with food (to reduce nausea).

  • Drugs that cause a serious reaction with Hepcvir: the antiarrhythmic amiodarone can cause severe bradycardia (slow heartbeat)
  • Drugs that affect Hepcvir: anticonvulsants like carbamazepine and phenytoin, the antibacterial rifampicin
  • Drugs that are affected by Hepcvir: some antivirals for HIV like tenofovir

Your weight and the genotype of Hepatitus C Virus that has been contracted will be important factors in determining your dosage. Only take the dossage that has been prescribed by your doctor. Commonly, a starting dose of 400 mg per day is prescribed. This may be increased to 600 mg per day depending on how the treatment is progressing. Do not adjust the dosage up or down by yourself.
