Azithromycin is to be made available over the counter to treat asymptomatic chlamydia infection, the Medicines and Healthcare products Regulatory Agency announced today (6 August 2008). It is the first oral antibiotic to be switched from prescription-only to pharmacy medicine status.

The drug will be available as Clamelle for people over 16 years of age who have tested positive for chlamydia but have no symptoms, and for their sexual partners.

Who may buy azithromycin?

Pharmacists will be able to sell azithromycin only to individuals with a positive NAAT (nucleic acid amplification test) result and to their sexual partners. Partner notification contact slips will be used to obtain a supply; partners will not need to have a positive test. A database of test results will be held, which pharmacists will need to access to confirm the results for index cases.

Manufacturer Actavis is working with the NPA on training materials for pharmacists and medicines counter assistants as well as a Clamelle-branded chlamydia test kit. The NPA has developed a chlamydia screening service package, which is due to be launched on 11 August 2008.

How is azithromycin taken?

Chlamydia treatment with azithromycin is very simple. You will take two 500mg tablets of azithromycin at the same time. In combination, the two tablets contain 1000 milligrams (1 gram) of the active ingredient, which is sufficient to cure your infection. You won’t need to take any further tablets afterwards.

Which dosage do I need to take?

Your pharmacist may provide you with four tablets instead of two, which contain a dosage of 250mg each. In this case, you must take all four tablets in order to take the 1000 mg dose of azithromycin required for chlamydia treatment.

Azithromycin tablets should be swallowed whole with a drink of water, either one hour before the next meal or at least two hours after the last meal. Your chlamydia treatment won’t affect your ability to drive a car or operate machinery.

Will the infection come back?

One course of azithromycin will clear the infection. Cases in which one course does not suffice are very rare. However, if you still have chlamydia symptoms one week after your treatment please consult a doctor. You may require further treatment or an additional test to establish the cause of your symptoms.

The most important precaution in order to prevent re-infection is to avoid sex for one week after your treatment. This is to ensure, that you do not pass your infection to a partner, who is likely to pass it back to you. If despite this recommendation you decide to have intercourse within one week of treatment, you must ensure you use a condom to minimise the risk of infection.

Can I drink alcohol with azithromycin?

Azithromycin does not interact with alcohol. Drinking alcohol won’t stop your treatment from working.

How does it work?

Azithromycin is an antibiotic which is used to treat a range of bacterial infections. These bacteria need certain proteins, without which they can’t grow and reproduce. Azithromycin prevents the bacteria from producing the proteins, thus preventing them from multiplying. As a result, the infection won’t spread further and your immune system can kill the remaining bacteria.
