Description domperidone

Domperidone is an effective anti-emetic (medicine used to prevent vomiting and nausea). It increases the intestinal movements and facilitates emptying of the bowel.

Effects in Breastfed Infants

One paper reported 2 studies. In one, 8 women received domperidone 10 mg 3 times daily from day 2 to 5 postpartum. In the other, 9 women received domperidone 10 mg 3 times daily for 10 days from week 2 postpartum. No side effects were reported in any of the breastfed (extent not stated) infants.

Eleven women took domperidone 10 mg 3 times daily for 7 days to increase the supply of pumped milk for their preterm neonates. No side effects were reported in their infants.

In a study of 90 mothers who received domperidone 10 mg three times daily for 2 or 4 weeks while providing milk for their preterm infants, there was no apparent difference in the frequency or types of adverse events that occurred in their infants, whether taking the active drug or placebo.

Side Effects of Domperidone

  • As with all medications, side effects are possible, and many have been reported with domperidone (textbooks often list any side effect ever reported, but symptoms reported are not necessarily due to the drug a person is taking). There is no such thing as a 100% safe drug. However, our clinical experience has been that side effects in the mother are extremely uncommon, except for increasing milk supply. Some side effects which mothers we have treated have reported (very uncommonly, incidentally):headache which disappeared after a few days or when the dose was reduced (probably the most common side effect)
  • abdominal cramps
  • dry mouth
  • alteration of menstrual periods, usually stopping them, but also breakthrough bleeding is possible. It is usual when breastfeeding not to have menstrual periods for many months
  • a very few mothers who have taken domperidone for many months, usually more than a year, have reported feelings of anxiety, sleeplessness, loss of appetite and other symptoms when they stopped the domperidone “cold turkey”.

Are There Long Term Concerns About The Use of Domperidone?

The manufacturer states in its literature that chronic treatment with domperidone in rodents has resulted in increased numbers of breast tumours in the rodents. The literature goes on to state that this has never been documented in humans. Note that toxicity studies of medication usually require treatment with huge doses over periods of time involving most or all of the animal’s lifetime. Note also that notbreastfeeding increases the risk of breast cancer, and breast cancer risk decreases the longer you breastfeed. Also note, in Canada we have used Domperidone as a ‘milk-making’ medication for over 20 years.

Warnings

Pregnancy - This medicine is not recommended for use in pregnant women unless the potential benefits outweigh the risks involved. The effect of this medicine on the fetus is not clearly established and hence you are advised to consult a doctor before taking this medicine.

Breast-feeding - This medicine is not recommended for use by breastfeeding mothers as there is a risk of side effects on the infant. You are advised to consult your doctors and consider the potential benefit and risks before consuming this medication.

Other medicines - Domperidone is known to interact with a large number of medications. You are advised to inform the usage of any medication including supplements and herbs to the doctor.

Pediatric use - Domperidone should not be used in children who are underweight or malnourished. It should not be used in children who weigh less than 35 kg.

Use in elderly - Domperidone should be used with caution in the elderly as the risk of adverse effects are very high.

Effect on heart - Domperidone can effect the functioning of the heart, especially in the elderly and people having an active condition like rhythm disorders and Congestive heart failure.

Electrolyte imbalance - Domperidone should be used with extreme caution if you are suffering from acute or long-term electrolyte imbalance in the body.
